Overview

Donor Lymphocyte Infusion (DLI) is an adoptive cellular immunotherapy used primarily to treat or prevent relapse after allogeneic hematopoietic stem cell transplantation (HSCT). In the specific "Two-Step" haploidentical transplant protocol developed by Northwell Health (based on methodology from Thomas Jefferson University), DLI is administered as the first step to provide early immune reconstitution and anti-leukemic activity through a graft-versus-tumor (GVT) effect. This is followed by the administration of cyclophosphamide to induce immune tolerance by selectively depleting alloreactive T-cells, and subsequently, the infusion of CD34+ selected donor stem cells. This regimen is designed for children and young adults with high-risk hematologic malignancies, including acute lymphoblastic leukemia (ALL), acute myeloid leukemia (AML), myelodysplastic syndromes (MDS), chronic myeloid leukemia (CML), and various lymphomas, who lack a suitably matched related or unrelated donor.
