This therapeutic approach, developed by the University of Liege, involves the infusion of donor-derived regulatory T cells (Tregs) for the treatment of steroid-refractory chronic graft-versus-host disease (cGVHD). The protocol utilizes CD4+CD25+CD127low Tregs isolated from the original stem cell donor, which are infused into the patient to restore immune tolerance and suppress the inflammatory processes driving cGVHD. To support the survival, stability, and in vivo expansion of these infused cells, the therapy is combined with rapamycin (sirolimus), an mTOR inhibitor that selectively promotes Treg survival over effector T cells, and low-dose interleukin-2 (IL-2), which provides essential growth signals through the high-affinity IL-2 receptor (CD25) constitutively expressed on Tregs. This combination strategy aims to achieve long-term immune homeostasis in patients who have failed standard corticosteroid therapy.
