Overview

Doravirine + islatravir is an investigational, once-daily, oral, two-drug fixed-dose combination regimen for the treatment of HIV-1 infection in adults. Doravirine is a non-nucleoside reverse transcriptase inhibitor (NNRTI) that inhibits HIV-1 reverse transcriptase by binding to a distinct site and blocking DNA polymerization. Islatravir is a first-in-class nucleoside reverse transcriptase translocation inhibitor (NRTTI), which acts as an immediate and delayed chain terminator during viral DNA synthesis by inhibiting the translocation step of reverse transcription. The combination aims to provide potent antiviral activity with a high barrier to resistance and favorable safety profile. Developed by Merck, this regimen has demonstrated non-inferiority to standard three-drug antiretroviral therapies in phase 3 trials for virologically suppressed adults with HIV-1[3][4][6][7].
