Overview

Dostarlimab + capecitabine + oxaliplatin is an experimental combination of an immune checkpoint inhibitor (dostarlimab) with two chemotherapeutic agents (capecitabine and oxaliplatin), currently under investigation as neoadjuvant therapy in colon cancer and potentially other solid tumors. Dostarlimab is a humanized monoclonal antibody targeting PD-1 (programmed death-1), thereby enhancing T-cell mediated anti-tumor responses. Capecitabine is an orally administered prodrug of 5-fluorouracil (5-FU) that interferes with DNA synthesis, and oxaliplatin is a platinum-based agent causing DNA crosslinking and apoptosis in rapidly dividing cells. The rationale for this combination is to provide both direct cytotoxic effects (from chemotherapy) and to augment anti-tumor immunity (via PD-1 blockade). This triplet regimen is being studied primarily for colon cancer, especially mismatch repair proficient/microsatellite stable (MMRp/MSS) types, in the neoadjuvant (pre-surgical) setting[1][2][4][6].
