Overview

This combination chemotherapy regimen, commonly known as the MAP protocol, consists of three active ingredients: methotrexate, doxorubicin (Adriamycin), and cisplatin. It is a standard-of-care treatment for osteosarcoma, particularly in pediatric and young adult populations. The N.N. Blokhin National Medical Research Center of Oncology (Blokhin's Russian Cancer Research Center) is conducting clinical trials (such as NCT05057130) to evaluate the efficacy and safety of this three-component neoadjuvant regimen compared to a two-component (doxorubicin and cisplatin) regimen in adult patients aged 24-40 years. The regimen aims to increase tumor necrosis and improve response rates. Doxorubicin acts by intercalating DNA and inhibiting topoisomerase II, cisplatin forms DNA crosslinks, and methotrexate inhibits dihydrofolate reductase, collectively disrupting DNA synthesis and repair in rapidly dividing cancer cells.
