AC4-CDDP4 is a sequential neoadjuvant chemotherapy regimen investigated in the PACER clinical trial (NCT02001519) for the treatment of triple-negative breast cancer (TNBC). The regimen consists of four cycles of Adriamycin (doxorubicin) and cyclophosphamide (AC4), followed by four additional cycles of cisplatin (CDDP4). This sequential approach was specifically evaluated in patients who did not achieve a clinical complete response after the initial AC cycles. Doxorubicin is an anthracycline that intercalates DNA and inhibits topoisomerase II; cyclophosphamide is an alkylating agent that cross-links DNA; and cisplatin is a platinum-based agent that forms DNA adducts, collectively inhibiting DNA replication and inducing apoptosis in rapidly dividing cancer cells. The trial was sponsored by the Asan Medical Center in South Korea.
