Overview

A combination chemotherapy regimen consisting of **doxorubicin**, **cyclophosphamide**, and **THP** (tetrahydropyranyl adriamycin, also known as pirarubicin). This regimen leverages the distinct mechanisms of each drug: - **Doxorubicin** is an anthracycline that intercalates DNA, inhibits topoisomerase II, and generates free radicals, leading to DNA damage and cell death. - **Cyclophosphamide** is an alkylating agent that crosslinks DNA, preventing cell division and leading to apoptosis. - **THP (pirarubicin)** is also an anthracycline with a similar mechanism to doxorubicin but with a distinct pharmacokinetic and toxicity profile, often considered less cardiotoxic. Combination regimens containing these drugs are primarily used in the treatment of various cancers, especially aggressive lymphomas and breast cancer. In Japan and some other regions, THP has been used as a substitute for doxorubicin in elderly patients due to its lower cardiotoxicity. The regimen can be considered an alternative to CHOP (in which doxorubicin is replaced by THP) or part of an anthracycline-containing regimen for solid tumors or hematological malignancies.[3]
