Overview

This is a combination chemotherapy regimen consisting of four drugs: - **Doxorubicin** (an anthracycline antibiotic that intercalates DNA and inhibits topoisomerase II) - **Cyclophosphamide** (an alkylating agent that crosslinks DNA) - **Vindesine** (a vinca alkaloid that inhibits microtubule assembly) - **Valproic acid** (a histone deacetylase inhibitor with epigenetic modulatory effects) The combination has been studied as a salvage therapy for patients with refractory or relapsing small cell lung cancer (SCLC) after failure of platinum-based regimens. Valproic acid is included to enhance the cytotoxicity of the VAC regimen through epigenetic modulation and increased apoptosis in tumor cells. The mechanism involves both intrinsic and extrinsic apoptotic pathways via caspase activation. This regimen has shown some activity in phase II clinical trials but did not significantly improve progression-free survival compared to historical controls[1][2][3].
