Overview

The AI regimen is a combination chemotherapy treatment consisting of doxorubicin (Adriamycin) and ifosfamide, primarily used for the treatment of soft tissue sarcomas. Doxorubicin is an anthracycline antibiotic that exerts its antineoplastic effect by intercalating between DNA base pairs and inhibiting topoisomerase II, leading to DNA strand breaks and inhibition of macromolecular synthesis. Ifosfamide is a nitrogen mustard alkylating agent that requires metabolic activation to form DNA-reactive species that cross-link DNA strands, thereby interfering with DNA replication and transcription. Fujian Medical University Union Hospital is investigating this regimen in a Phase 2 clinical trial as part of a total neoadjuvant therapy approach, combined with hypofractionated radiotherapy, for patients with localized extremity soft tissue sarcomas.
