Doxorubicin + sorafenib is a combination of two anticancer agents used primarily in clinical research for the treatment of advanced hepatocellular carcinoma (HCC). Doxorubicin is an anthracycline antibiotic that acts as a cytotoxic agent by intercalating DNA and generating reactive oxygen species, leading to DNA damage and apoptosis. Sorafenib is an oral multikinase inhibitor targeting several kinases involved in tumor cell proliferation and angiogenesis, including RAF kinases and vascular endothelial growth factor receptors. The rationale for combining these drugs is to enhance antitumor efficacy by attacking cancer cells through complementary mechanisms—doxorubicin’s direct cytotoxicity and sorafenib’s inhibition of tumor growth signaling pathways and angiogenesis. Clinical trials have shown that while the combination can inhibit tumor growth more than either agent alone in preclinical models, large phase III studies did not demonstrate improved overall survival or progression-free survival compared to single-agent therapy with increased toxicity observed[1][5][6][7].
