Overview

This combination therapy, investigated by Tanta University, consists of doxycycline, pentoxifylline, and nitazoxanide. Doxycycline is a second-generation tetracycline antibiotic that inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit. Pentoxifylline is a xanthine derivative and non-selective phosphodiesterase inhibitor used for its vasoactive and immunomodulatory effects, specifically its ability to inhibit TNF-alpha production. Nitazoxanide is a thiazolide antimicrobial that interferes with the pyruvate:ferredoxin oxidoreductase (PFOR) enzyme-dependent electron transfer reaction, essential for anaerobic metabolism. The combination is being evaluated in a Phase 2/3 trial for the treatment of untreated gonorrhea and chlamydia infections in sexually active men to determine if the triple therapy is superior to doxycycline monotherapy.
