Overview

**doxycycline + rifampicin + levofloxacin** is a triple-antibiotic combination therapy being investigated for the treatment of brucellosis, including uncomplicated and osteoarticular forms. The regimen combines three distinct mechanisms of action: **doxycycline** (a tetracycline) inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit; **rifampicin** (a rifamycin) inhibits DNA-dependent RNA polymerase, blocking transcription; and **levofloxacin** (a fluoroquinolone) inhibits DNA gyrase and topoisomerase IV, preventing DNA replication and repair. This combination is specifically designed to improve outcomes in osteoarticular brucellosis due to the superior bone and joint penetration of fluoroquinolones and to reduce the risk of antimicrobial resistance associated with rifampicin-based dual therapies. It is currently being evaluated in the multicenter BRUCE study led by The First Affiliated Hospital of Shihezi University.
