Overview

Droxidopa + carbidopa is a theoretical or off-label drug combination that pairs two agents commonly used in the management of neurogenic orthostatic hypotension and Parkinson's disease. Droxidopa is a synthetic amino acid precursor of norepinephrine that increases blood pressure by being converted to norepinephrine via aromatic L-amino acid decarboxylase after crossing the blood-brain barrier[4][8]. It is primarily indicated to treat neurogenic orthostatic hypotension associated with conditions such as Parkinson's disease and multiple system atrophy[1][5]. Carbidopa is a peripheral dOPA-decarboxylase inhibitor that prevents the conversion of levodOPA or other precursors like droxidOPA to dopamine/norepinephrine outside the central nervous system; thus it can increase central availability of these neurotransmitters when co-administered[3]. While there are no fixed-dose combinations marketed specifically as "droxidopa + carbidopa," both drugs may be used together in clinical practice—especially in patients with Parkinson’s disease who require both symptomatic management of motor symptoms and treatment of autonomic dysfunction.
