DTRMWXHS-12 + everolimus

DTRMWXHS-12 + everolimus is an investigational oral combination therapy under clinical development for patients with relapsed or refractory lymphoid malignancies, most notably chronic lymphocytic leukemia (CLL), diffuse large B-cell lymphoma (DLBCL), and other B-cell lymphomas. DTRMWXHS-12 is a novel Bruton tyrosine kinase (BTK) inhibitor, while everolimus is a small molecule inhibitor of the mammalian target of rapamycin (mTOR). The rationale behind the combination arises from synergistic preclinical activity seen with simultaneous BTK and mTOR pathway inhibition, which can enhance apoptotic signaling and disrupt survival/proliferative processes in malignant B cells. DTRMWXHS-12 selectively antagonizes BTK, a signaling node essential for B-cell receptor signaling and survival; everolimus inhibits mTOR, affecting cell metabolism, growth, proliferation, and angiogenesis. This doublet regimen is being developed as part of both dual and triplet combination protocols (e.g., DTRM-555, which adds pomalidomide), aiming to optimize efficacy while maintaining tolerability in heavily pretreated patients[3][7][9].
