duloxetine + amitriptyline

Duloxetine + amitriptyline is an investigational combination therapy being evaluated for the treatment of chemotherapy-induced peripheral neuropathy (CIPN). Duloxetine is a serotonin-norepinephrine reuptake inhibitor (SNRI) that increases the levels of these neurotransmitters in the spinal cord to enhance descending inhibitory pain pathways. Amitriptyline is a tricyclic antidepressant (TCA) that also inhibits the reuptake of serotonin and norepinephrine but possesses a broader pharmacological profile, including the blockade of voltage-gated sodium channels, NMDA receptors, and various G-protein coupled receptors (muscarinic, histaminergic, and adrenergic). This combination is being studied by Beni-Suef University in a Phase 2 clinical trial (NCT06091553) to determine if the addition of low-dose amitriptyline to a standard duloxetine regimen provides superior efficacy and safety compared to duloxetine monotherapy in cancer patients with grade 1 or higher sensory CIPN.
