This protocol utilizes a tailored approach where the choice and duration of therapy are guided by clarithromycin resistance patterns identified via dual priming oligonucleotide-polymerase chain reaction (DPO-PCR). Patients with susceptible strains are treated with triple therapy (typically a combination of a proton pump inhibitor, amoxicillin, and clarithromycin) for either 7 or 14 days. Patients with resistant strains receive bismuth quadruple therapy (typically a combination of a proton pump inhibitor, bismuth subcitrate, metronidazole, and tetracycline) for either 7 or 14 days.
