Overview

**Durvalumab + daratumumab** is a combination of two monoclonal antibodies designed for the treatment of relapsed and refractory multiple myeloma. Durvalumab is an immune checkpoint inhibitor targeting PD-L1, leading to enhanced T-cell-mediated anti-tumor immune responses. Daratumumab targets CD38, a protein overexpressed in multiple myeloma cells, resulting in direct cytotoxicity and immunomodulatory effects by reducing immunosuppressive cell populations (e.g., Tregs, myeloid-derived suppressor cells). The combination was studied in phase 2 clinical trials for patients resistant to daratumumab, aiming to overcome resistance by simultaneously blocking PD-L1/PD-1 signaling and further modulating the immune microenvironment. While this approach demonstrated acceptable safety and reductions in regulatory T-cell frequency, clinical efficacy was not observed, and no patients achieved partial response or better[1][3][5][7].
