This is a combination regimen consisting of the monoclonal antibody durvalumab, the topoisomerase II inhibitor etoposide, and two platinum-based chemotherapeutic agents, carboplatin and cisplatin. Durvalumab is a selective, high-affinity human IgG1 monoclonal antibody that blocks programmed death-ligand 1 (PD-L1) from binding to PD-1 and CD80, thereby enhancing anti-tumor immune responses. Etoposide inhibits DNA synthesis by forming a complex with topoisomerase II and DNA. Carboplatin and cisplatin are alkylating agents that cause DNA crosslinking and apoptosis in rapidly dividing cells. This combination is primarily used as first-line treatment for extensive-stage small cell lung cancer (ES-SCLC), where it has demonstrated improved overall survival compared to chemotherapy alone[1][4][5][9].
