Drug intelligence / Profile preview
Imfinzi + etoposide + platinum-based chemotherapy is a combination chemo-immunotherapy regimen used as a first-line treatment for extensive-stage small cell lung cancer (ES-SCLC). The regimen consists of durvalumab (Imfinzi), a human IgG1 kappa monoclonal antibody that targets programmed death-ligand 1 (PD-L1); etoposide, a topoisomerase II inhibitor; and a platinum-based chemotherapeutic agent, which is typically either carboplatin or cisplatin. Durvalumab works by blocking the interaction of PD-L1 with PD-1 and CD80, thereby releasing T-cell inhibition and enhancing the anti-tumor immune response. Etoposide and the platinum agents act as conventional cytotoxic chemotherapies; etoposide prevents cell division by forming a complex with topoisomerase II and DNA, while carboplatin and cisplatin cause DNA crosslinking and induce apoptosis in rapidly dividing cells. This combination was approved by the FDA in March 2020 based on the Phase III CASPIAN trial, which demonstrated a significant improvement in overall survival compared to chemotherapy alone.
