Durvalumab + gemcitabine + cisplatin + nab-paclitaxel is a combination chemo-immunotherapy regimen being investigated as a neoadjuvant treatment for resectable biliary tract cancer (BTC). Durvalumab is a human monoclonal antibody that targets programmed death-ligand 1 (PD-L1), preventing its interaction with PD-1 and CD80 to enhance T-cell mediated anti-tumor activity. Gemcitabine and cisplatin are standard cytotoxic agents that inhibit DNA synthesis and cause DNA cross-linking, respectively. Nab-paclitaxel is an albumin-bound nanoparticle formulation of paclitaxel that stabilizes microtubules, leading to cell cycle arrest. This specific four-drug regimen is being evaluated in a Phase II trial led by Tianjin Medical University Cancer Institute and Hospital to improve surgical outcomes and survival in patients with operable BTC, building upon the established efficacy of durvalumab combinations in the metastatic setting.
