Overview

Durvalumab + monalizumab is a combination of two monoclonal antibodies used in cancer immunotherapy, particularly for advanced solid tumors and non-small cell lung cancer (NSCLC). Durvalumab is a high-affinity, human IgG1 monoclonal antibody that targets programmed death ligand 1 (PD-L1), blocking its interaction with PD-1 and CD80 on T cells. This blockade lifts the inhibition on T-cell activation, enabling immune-mediated tumor cell killing. Monalizumab is a first-in-class, humanized IgG4 monoclonal antibody that targets NKG2A receptor, an inhibitory receptor expressed on natural killer (NK) cells and CD8+ T cells. By preventing NKG2A receptor from binding to HLA-E on tumor cells, monalizumab reduces inhibitory signaling and enhances both innate and adaptive antitumor immunity. The combination aims to simultaneously block two distinct immune checkpoint pathways—PD-L1/PD-1/CD80 and NKG2A receptor/HLA-E—to boost anti-tumor responses beyond what either agent achieves alone[1][3][4][6][8].
