Durvalumab and pembrolizumab are immune checkpoint inhibitors (ICIs) being evaluated as maintenance therapy for advanced biliary tract cancer (BTC) in a Phase IIb randomized clinical trial led by Yonsei University. Durvalumab is a human monoclonal antibody that blocks programmed death-ligand 1 (PD-L1), while pembrolizumab is a humanized monoclonal antibody that targets programmed cell death protein 1 (PD-1). In the trial context, these agents are used either as monotherapy or in combination with other targeted therapies (such as lenvatinib or venadaparib) or cytokines (SLC-3010) to maintain disease control in patients who have previously responded to first-line chemoimmunotherapy. By inhibiting the PD-1/PD-L1 pathway, these drugs aim to restore the anti-tumor activity of T cells within the immunosuppressive microenvironment of biliary tract malignancies.
