The combination of durvalumab, pertuzumab, and trastuzumab is being investigated as a treatment regimen for HER2-positive breast cancer. This combination brings together three distinct mechanisms of action to target HER2-positive cancer cells. ## Mechanism of Action This combination therapy works through multiple complementary mechanisms: 1. **Durvalumab** is an immune checkpoint inhibitor that activates the immune system to attack cancer cells. 2. **Pertuzumab** binds to subdomain II of HER2 receptors and blocks ligand-dependent heterodimerization with other HER family receptors (HER1, HER3, and HER4), inhibiting downstream signaling pathways and suppressing cell proliferation[8]. 3. **Trastuzumab** binds to subdomain IV of HER2 and disrupts ligand-independent HER2 signaling, blocks the generation of active p95HER2 fragments by inhibiting HER2 cleavage, and stimulates antibody-dependent cell-mediated cytotoxicity (ADCC)[6][8]. Together, these agents provide a dual blockade of HER2-driven signaling pathways while also activating the immune system against cancer cells[1][3]. ## Clinical Development This combination is being investigated in a multicenter Phase II trial (DTP trial) for HER2-enriched breast cancer[4]. The trial is evaluating the safety and efficacy of durvalumab used in combination with trastuzumab and pertuzumab. In this trial, patients receive: - Durvalumab at a fixed dose of 1120 mg IV - Trastuzumab at 8 mg/kg IV loading dose, followed by 6 mg/kg - Pertuzumab at 840 mg IV loading dose, followed by 420 mg These medications are administered on Day 1 every 3 weeks for 6 cycles[1][4]. The primary outcome measure is pathological complete response rate (pCR), with secondary outcomes including pCR by percentage of tumor-infiltrating lymphocytes, pCR by PD-L1 status, 3-year disease-free survival, and treatment-related adverse events[4]. The trial is being conducted by the Methodist Hospital Research Institute in collaboration with AstraZeneca[4].
