Drug intelligence / Profile preview
The combination of durvalumab, platinum chemotherapy (either cisplatin or carboplatin), and etoposide is a standard-of-care chemoimmunotherapy regimen primarily indicated for the first-line treatment of extensive-stage small cell lung cancer (ES-SCLC). Durvalumab is a human IgG1 monoclonal antibody that targets programmed cell death ligand 1 (PD-L1), blocking its interaction with PD-1 and CD80 to restore T-cell-mediated anti-tumor immune responses. Etoposide is a semisynthetic podophyllotoxin derivative that inhibits topoisomerase II, preventing DNA re-ligation and inducing double-strand breaks. The platinum agents (cisplatin or carboplatin) act as DNA alkylating agents, forming covalent intrastrand and interstrand cross-links that disrupt DNA replication and transcription. Together, this combination leverages the cytotoxic effects of chemotherapy to release tumor antigens and synergize with immune checkpoint inhibition, significantly improving overall survival as demonstrated in the Phase III CASPIAN trial.
