Overview

Durvalumab + trametinib is an investigational combination therapy consisting of two agents with distinct mechanisms of action. Durvalumab is a human monoclonal antibody that targets programmed death-ligand 1 (PD-L1), blocking its interaction with PD-1 and CD80 to enhance T-cell mediated immune responses against tumor cells. Trametinib is a small molecule inhibitor of mitogen-activated protein kinase kinase (MEK) 1 and 2, components of the MAPK/ERK pathway involved in cell proliferation and survival. The rationale for combining these drugs is to leverage MEK inhibition to modulate the tumor microenvironment and potentially increase the efficacy of immune checkpoint blockade by enhancing T-cell activation or infiltration. This combination has been studied in phase II clinical trials for metastatic colorectal cancer (MSS mCRC), where it was found to be tolerable but showed limited clinical activity[5].
