Overview

This is a four-drug combination regimen consisting of two immune checkpoint inhibitors—**durvalumab** (an anti-PD-L1 monoclonal antibody) and **tremelimumab** (an anti-CTLA-4 monoclonal antibody)—plus two cytotoxic chemotherapies: **gemcitabine** (a nucleoside analog that inhibits DNA synthesis) and **carboplatin** (a platinum-based alkylating agent). Durvalumab blocks PD-L1 on tumor and immune cells to enhance T-cell responses. Tremelimumab blocks CTLA-4, further promoting T-cell activation and proliferation. Gemcitabine and carboplatin work by damaging tumor DNA, with gemcitabine interrupting DNA synthesis and carboplatin crosslinking DNA strands. The combination aims to synergize immunotherapy's effects with the cytotoxic action of chemotherapy. The regimen is primarily under investigation or approved in solid tumors, particularly **non-small cell lung cancer (NSCLC)**, but also studied in **biliary tract cancer**, **triple-negative breast cancer**, and other advanced malignancies[5][7][8].
