Overview

This is a combination therapy consisting of **durvalumab** (an anti-PD-L1 monoclonal antibody), **tremelimumab** (an anti-CTLA-4 monoclonal antibody), **nab-paclitaxel** (albumin-bound paclitaxel, a microtubule inhibitor), and **gemcitabine** (a nucleoside analog inhibitor of DNA synthesis). Durvalumab blocks PD-L1, preventing it from binding and inactivating immune T cells. Tremelimumab blocks CTLA-4, a negative regulator of T cell activation, promoting enhanced immune response against tumor cells. Nab-paclitaxel and gemcitabine provide cytotoxic chemotherapy, disrupting cell division and DNA replication, respectively. The combination is under investigation primarily as a first-line therapy in **metastatic pancreatic ductal adenocarcinoma**, aiming to synergize immune checkpoint inhibition with cytotoxic chemotherapy. The combination was evaluated in randomized phase II trials and demonstrated manageable toxicity but did not significantly improve overall survival compared to chemotherapy alone[1][2][4][5].
