Overview

A combination immunotherapy-chemotherapy regimen consisting of durvalumab (PD-L1 inhibitor), tremelimumab (CTLA-4 inhibitor), and pemetrexed (antifolate chemotherapy). This regimen is primarily used for the treatment of metastatic non-small cell lung cancer (NSCLC). The combination works through complementary mechanisms: durvalumab enhances T-cell function through PD-L1 blockade, tremelimumab diversifies T-cell responses by targeting CTLA-4, and pemetrexed stops cancer cells from using folic acid to make DNA, causing cell death. The FDA approved this combination in November 2022 based on the POSEIDON clinical trial, which demonstrated significant improvements in overall survival and progression-free survival compared to chemotherapy alone.
