durvalumab + tremelimumab + Yttrium-90 radioembolisation is a combination therapy regimen being evaluated by The University of Hong Kong for the treatment of metastatic hepatocellular carcinoma (HCC). The regimen consists of two immune checkpoint inhibitors developed by AstraZeneca—durvalumab (an anti-PD-L1 monoclonal antibody) and tremelimumab (an anti-CTLA-4 monoclonal antibody)—alongside Yttrium-90 (Y90) radioembolisation, a form of selective internal radiation therapy (SIRT). This triple combination aims to leverage the synergistic potential of dual checkpoint blockade and localized radiation to induce an abscopal effect, enhancing systemic anti-tumor immunity in patients with previously untreated advanced disease. The clinical trial (NCT05809869) utilizes a priming dose of immunotherapy followed by Y90 administration to optimize the immune-priming effects of radiation.
