This combination therapy involves the use of an immune checkpoint inhibitor (either the PD-L1 inhibitor durvalumab or the PD-1 inhibitor pembrolizumab) in conjunction with lenvatinib, a multi-receptor tyrosine kinase inhibitor. Developed in the context of a Phase IIb randomized clinical trial by Yonsei University, this regimen is being evaluated as a maintenance therapy for patients with advanced biliary tract cancer (BTC) who have achieved disease control following initial chemoimmunotherapy (gemcitabine, cisplatin, and an ICI). Lenvatinib's inhibition of VEGFR and FGFR pathways is hypothesized to modulate the tumor immune microenvironment, potentially overcoming resistance and enhancing the anti-tumor activity of the PD-1/PD-L1 blockade.
