Overview

durvalumab or pembrolizumab + venadaparib is a combination immunotherapy and targeted therapy regimen being investigated as a maintenance treatment for advanced biliary tract cancer (BTC). This regimen combines an immune checkpoint inhibitor (ICI)—either the PD-L1 inhibitor durvalumab or the PD-1 inhibitor pembrolizumab—with venadaparib (IDX-1197), a potent small-molecule inhibitor of poly (ADP-ribose) polymerase 1 and 2 (PARP1/2). The combination is specifically being studied by Yonsei University in a Phase IIb trial for patients who have achieved disease control after initial chemoimmunotherapy with gemcitabine, cisplatin, and an ICI. The therapeutic rationale is based on the potential for PARP inhibitors to enhance the efficacy of ICIs by increasing DNA damage, promoting neoantigen release, and modulating the tumor immune microenvironment, particularly in tumors with homologous recombination deficiencies or platinum sensitivity.
