Dutasteride + ethanol is a topical formulation of the dual 5-alpha-reductase inhibitor dutasteride, primarily investigated for the treatment of androgenetic alopecia (male pattern hair loss). In this formulation, ethanol serves as a solvent and penetration enhancer, facilitating the delivery of the relatively large dutasteride molecule (MW 528.5) through the stratum corneum into the hair follicles. By inhibiting both Type 1 and Type 2 isoforms of 3-oxo-5-alpha-steroid 4-dehydrogenase, the drug reduces the local conversion of testosterone to dihydrotestosterone (DHT), the primary androgen responsible for follicle miniaturization. Topical administration is intended to provide localized therapeutic effects while minimizing systemic exposure and the associated side effects, such as sexual dysfunction, that are commonly observed with oral 5-alpha-reductase inhibitors.
