Duvelisib + pembrolizumab is an experimental combination therapy consisting of duvelisib, a small molecule dual inhibitor of phosphoinositide 3-kinase (PI3K)-delta and PI3K-gamma isoforms, and pembrolizumab, a monoclonal antibody immune checkpoint inhibitor targeting the programmed cell death protein 1 (PD-1) receptor. Duvelisib inhibits signaling pathways key to B-cell malignancies and modulates tumor-associated macrophages and T-cell populations, while pembrolizumab boosts immune-mediated antitumor activity by blocking the PD-1 pathway. This combination was investigated for synergistic immunomodulatory and antitumor effects, particularly in advanced head and neck squamous cell carcinoma.
