A combination therapy of **eciskafusp alfa** and **BCG (Bacillus Calmette-Guérin)** developed for the treatment of BCG-unresponsive high-risk non-muscle invasive bladder cancer (NMIBC). **Eciskafusp alfa** is an experimental immunocytokine designed to target the PD-1 pathway, delivering an IL-2 variant (IL-2v) preferentially to PD-1–positive immune cells, aiming to enhance anti-tumor immunity within the bladder. **BCG** is an established intravesical immunotherapy employing live attenuated Mycobacterium bovis to stimulate a local immune response against bladder tumors. The combination is administered intravesically to maximize local immunostimulatory effects and prevent recurrence or progression in NMIBC patients unresponsive to BCG monotherapy. The combination has not received regulatory approval and is being tested for safety, tolerability, pharmacokinetics, biomarkers, pharmacodynamics, and anti-tumor activity[1][2][3][4][5][6][8].
