Drug intelligence / Profile preview
**Ecnoglutide + metformin** is a combination therapy consisting of ecnoglutide (XW003), a novel, long-acting, cAMP-biased glucagon-like peptide-1 (GLP-1) receptor agonist, and metformin, a biguanide antihyperglycemic agent. Ecnoglutide, developed by Sciwind Biosciences and licensed to Pfizer in China, is engineered to preferentially activate the cAMP signaling pathway while minimizing β-arrestin recruitment, thereby reducing receptor internalization and desensitization to achieve sustained therapeutic efficacy with improved gastrointestinal tolerability. Metformin primarily decreases hepatic glucose production and improves insulin sensitivity. This combination is used for the treatment of type 2 diabetes mellitus (T2DM) and is particularly evaluated in patients whose blood glucose is inadequately controlled by metformin monotherapy. In the Phase III EECOH-2 trial, the co-administration of once-weekly ecnoglutide with metformin demonstrated superior glycemic control and weight reduction compared to dulaglutide plus metformin, with a favorable safety profile.
