Soliris (eculizumab) and Ultomiris (ravulizumab) are monoclonal antibodies that target the complement protein C5. Developed by Alexion Pharmaceuticals (now part of AstraZeneca), they inhibit the cleavage of C5 into C5a and C5b, thereby preventing the formation of the terminal complement complex (C5b-9). Soliris was the first-in-class C5 inhibitor, while Ultomiris is a second-generation, long-acting formulation designed with a higher binding affinity and a longer half-life, allowing for maintenance dosing every 8 weeks compared to every 2 weeks for Soliris. They are primarily used to treat complement-mediated diseases such as paroxysmal nocturnal hemoglobinuria (PNH), atypical hemolytic uremic syndrome (aHUS), generalized myasthenia gravis (gMG), and neuromyelitis optica spectrum disorder (NMOSD). Both drugs are available in the US only through a restricted access program called the Ultomiris and Soliris REMS due to the risk of serious meningococcal infections.
