edoxaban + LMWH

Edoxaban + LMWH is an intensive anticoagulation regimen being investigated in the Phase 3 HERO-19 trial for the treatment of hospitalized COVID-19 patients. The regimen consists of an initial in-hospital phase of therapeutic-dose low-molecular-weight heparin (LMWH) followed by edoxaban, a direct oral anticoagulant (DOAC). Edoxaban is a small molecule that selectively and reversibly inhibits Factor Xa, a key enzyme in the coagulation cascade. LMWH, a biologic agent derived from heparin, enhances the activity of antithrombin III to inhibit both Factor Xa and thrombin (Factor IIa). This sequential strategy is designed to mitigate COVID-19-associated coagulopathy, which significantly increases the risk of deep vein thrombosis and pulmonary embolism. The trial is sponsored by Universitätsklinikum Hamburg-Eppendorf, utilizing edoxaban originally developed by Daiichi Sankyo.
