Eflornithine + mitoguazone is a combination of two small molecule inhibitors developed for the treatment of malignant brain tumors, particularly recurrent anaplastic gliomas. Eflornithine (DFMO) is an irreversible inhibitor of ornithine decarboxylase, a key enzyme in polyamine biosynthesis that is essential for cell proliferation. Mitoguazone (MGBG) is a competitive inhibitor of S-adenosylmethionine decarboxylase, another enzyme involved in polyamine synthesis. By targeting two distinct steps in the polyamine pathway, this combination aims to more effectively suppress tumor growth than either agent alone. Clinical studies have shown that this regimen can be well-tolerated and may stabilize disease or induce responses in some patients with recurrent malignant gliomas who have failed prior therapies[1][3][7]. However, dose-limiting toxicities include gastrointestinal effects and myelosuppression.
