Drug intelligence / Profile preview
Eftilagimod alfa in combination with pembrolizumab and chemotherapy is an investigational chemo-immunotherapy regimen evaluated for the first-line treatment of advanced or metastatic non-small cell lung cancer (NSCLC). Eftilagimod alfa (efti) is a soluble LAG-3 fusion protein that acts as an antigen-presenting cell (APC) activator by binding to MHC class II molecules, thereby stimulating both innate and adaptive immune responses, including the activation of CD8+ cytotoxic T cells and CD4+ helper T cells. Pembrolizumab is a humanized monoclonal antibody targeting the programmed cell death protein 1 (PD-1) receptor, which blocks its interaction with PD-L1 and PD-L2 to unleash T-cell-mediated anti-tumor activity. The addition of standard platinum-based doublet chemotherapy (such as carboplatin/pemetrexed or carboplatin/paclitaxel) provides direct cytotoxic effects on tumor cells, releasing tumor antigens that further prime the immune response. Although early-phase trials like INSIGHT-003 demonstrated encouraging efficacy and survival benefits, the pivotal Phase III TACTI-004 trial evaluating this triplet regimen in first-line metastatic NSCLC was recommended to be halted in March 2026 following a planned interim futility analysis.
