Overview

Elenbecestat + digoxin is a drug combination primarily utilized in clinical pharmacology studies to evaluate drug-drug interactions. **Elenbecestat (E2609)** is a small molecule inhibitor of beta-amyloid converting enzyme 1 (BACE1), developed by **Eisai** and **Biogen** for the treatment of Alzheimer's disease. **Digoxin** is a cardiac glycoside that inhibits the sodium-potassium ATPase (Na+/K+-ATPase) and is a well-characterized substrate for the P-glycoprotein (P-gp) efflux transporter. The combination is typically studied to determine if elenbecestat acts as an inhibitor or inducer of P-gp, which would alter the pharmacokinetics and safety profile of digoxin. While digoxin remains an approved treatment for heart failure and atrial fibrillation, the clinical development of elenbecestat was discontinued in 2019 following Phase 3 trial results that did not support a favorable benefit-risk profile.
