A combination therapy that pairs eliglustat, a glucosylceramide synthase inhibitor, with immune checkpoint inhibitors. Eliglustat inhibits glycosphingolipid synthesis, which increases the exposure of major histocompatibility complex (MHC) and tumor antigen peptides, enhancing the antitumor response of CD8+ T cells. This mechanism complements immune checkpoint inhibitors, which block proteins like PD-1 that normally prevent T cells from attacking cancer cells. The combination has shown promising results in clinical trials for advanced cancers, particularly for proficient mismatch repair/microsatellite stable (pMMR/MSS) colorectal cancer.
