Overview

Elinogrel + aspirin is a theoretical combination of two antiplatelet agents, each with distinct mechanisms of action. *Elinogrel* is a P2Y12 receptor antagonist that inhibits ADP-induced platelet aggregation, similar in class to clopidogrel, but is not approved or marketed. *Aspirin* irreversibly inhibits cyclooxygenase-1 (COX-1), preventing thromboxane A2 synthesis and thereby reducing platelet aggregation. Dual antiplatelet therapy (DAPT) with agents acting on P2Y12 and COX-1 has been proven to reduce major adverse cardiovascular events such as myocardial infarction and stroke (most evidence is for clopidogrel + aspirin)[2][4], though at the cost of increased bleeding risk. Elinogrel was under development for acute coronary syndromes and other high-risk thrombosis situations, but its clinical development appears to be discontinued.
