Elobixibat + cholestyramine is a combination therapy under investigation for metabolic liver diseases, particularly nonalcoholic fatty liver disease (NAFLD) and metabolic dysfunction-associated steatohepatitis (MASH). Elobixibat is an inhibitor of the ileal bile acid transporter (IBAT), which blocks the reabsorption of bile acids in the terminal ileum, increasing their excretion and promoting colonic motility. Cholestyramine is a bile acid sequestrant that binds to bile acids in the intestine, preventing their reabsorption and increasing fecal excretion. Together, these drugs decrease recirculation of bile acids to the liver, stimulate new bile acid synthesis from cholesterol, reduce hepatic free cholesterol accumulation, lower LDL cholesterol levels, and improve hepatic lipid metabolism. This combination has shown additive effects on reducing serum lipids and improving markers of liver health in preclinical models and early clinical trials for NAFLD/MASH[1][4][6][8].
