Elranatamab + iberdomide is an investigational combination therapy being studied for the treatment of relapsed or refractory multiple myeloma. Elranatamab is a humanized bispecific monoclonal antibody targeting B-cell maturation antigen (BCMA) on myeloma cells and CD3 on T cells, thereby redirecting T cells to kill BCMA-expressing myeloma cells. Iberdomide is a novel cereblon E3 ligase modulator (CELMoD) with immunomodulatory and antimyeloma activity that induces tumor cell death and modulates the immune response. The combination leverages complementary mechanisms—immunomodulation by iberdomide and T-cell redirection by elranatamab—to potentially enhance antimyeloma activity. Both agents are given in cycles: elranatamab is administered subcutaneously, typically weekly, and iberdomide is taken orally once daily for 21 days of each 28-day treatment cycle. The combination is under evaluation in phase 1b clinical trials for safety, tolerability, and efficacy in this patient population[1][3][4][7].
