The GTP regimen is a combination therapy consisting of emapalumab, teniposide, and methylprednisolone, specifically being evaluated by Beijing Friendship Hospital for the treatment of refractory or recurrent hemophagocytic lymphohistiocytosis (HLH). Emapalumab is a monoclonal antibody that binds to and neutralizes interferon gamma (IFN-γ), a key cytokine that drives the hyperinflammation and immune dysregulation characteristic of HLH. Teniposide is a cytotoxic chemotherapy agent and a derivative of podophyllotoxin that acts as a topoisomerase II inhibitor, inducing DNA breaks and cell death in overactive immune cells. Methylprednisolone is a synthetic glucocorticoid that provides broad immunosuppressive and anti-inflammatory effects by activating the glucocorticoid receptor. This triple-combination approach is designed to rapidly control the life-threatening cytokine storm and eliminate the pathological immune cell populations in patients who have failed standard frontline therapies.
