Overview

The regimen centers on emavusertib (CA-4948), an oral small molecule inhibitor of Interleukin-1 Receptor-Associated Kinase 4 (IRAK4) and Fms-like tyrosine kinase 3 (FLT3), which is designed to inhibit the NFκB pathway and overcome resistance to standard therapies. Emavusertib is administered alongside the mFOLFOX7 chemotherapy backbone (comprising oxaliplatin, leucovorin, and 5-fluorouracil) and a PD-1 checkpoint inhibitor (either nivolumab or pembrolizumab). For patients with HER2-positive tumors, the regimen further incorporates the anti-HER2 monoclonal antibody trastuzumab. This multi-modal approach aims to synergize targeted kinase inhibition with cytotoxic chemotherapy and immune checkpoint blockade to improve outcomes in aggressive gastrointestinal malignancies.
