Empagliflozin + pioglitazone is a research combination therapy being investigated by The University of Texas Health Science Center at San Antonio for the treatment of type 2 diabetes mellitus. The combination consists of two small molecule active ingredients: empagliflozin, a sodium-glucose co-transporter-2 (SGLT2) inhibitor, and pioglitazone, a thiazolidinedione that acts as a peroxisome proliferator-activated receptor gamma (PPAR-gamma) agonist. This Phase 1 study specifically examines whether pioglitazone can block the empagliflozin-induced stimulation of endogenous glucose production, lipolysis, and ketone production in individuals with type 2 diabetes. Empagliflozin works by inhibiting glucose reabsorption in the kidneys to promote urinary glucose excretion, while pioglitazone improves insulin sensitivity in the liver and adipose tissue. Research from this institution suggests that coadministration may prevent the increase in plasma ketone concentrations often associated with SGLT2 inhibitor therapy.
