Mechanistically, empagliflozin promotes urinary glucose excretion by selectively inhibiting SGLT2 in the proximal renal tubules, while warfarin functions as an anticoagulant by inhibiting vitamin K epoxide reductase, thus reducing the synthesis of vitamin K–dependent clotting factors II, VII, IX, and X. There is evidence from controlled studies in healthy subjects showing no clinically significant pharmacokinetic or pharmacodynamic interaction between empagliflozin and warfarin, with co-administration not affecting exposure or activity of either drug, and no requirement for dose adjustment when used together[1][2].
