enalapril + spironolactone + carvedilol is a combination pharmacological regimen investigated for the treatment of chronic Chagas cardiomyopathy. The regimen consists of two renin-angiotensin system (RAS) inhibitors—enalapril (an ACE inhibitor) and spironolactone (a mineralocorticoid receptor antagonist)—and carvedilol, a non-selective beta-blocker with alpha-1 blocking activity. This therapeutic strategy, studied by the Federal University of Minas Gerais in the NCT01557140 trial, aims to mitigate the progressive cardiac damage and neurohormonal activation characteristic of Trypanosoma cruzi infection. Clinical findings suggest the combination is safe and effective in improving left ventricular ejection fraction and clinical status in patients with Chagas-related heart failure.
