Enamel matrix derivative (EMD) is a biologic regenerative material, primarily composed of amelogenins extracted from porcine embryonic enamel matrix, used in periodontal therapy to stimulate the regeneration of tooth-supporting tissues. In a clinical trial conducted by the University of São Paulo, EMD was evaluated in combination with acellular dermal matrix graft (ADMG) for the treatment of Miller Class I or II gingival recessions in smokers. The mechanism of action involves mimicking the natural process of tooth development by inducing mesenchymal cells to differentiate and promote the formation of new cementum, periodontal ligament, and alveolar bone. This combination therapy aims to enhance angiogenesis and fibroblast activity, which are often compromised in smokers, thereby improving the predictability and stability of root coverage procedures compared to graft-only techniques.
